I am therapist, I will be billing clients for services rendered. I will not be selling anything tangible. I will charge a fee for service. I do have a name for my business if I can do this without too much of a hassle. If it is more simple, I can just use my name.

Therapist may fall within personal services and may be taxable in your state and so you may need a sales tax id number.  Other than that, you will need a federal tax ID EIN, a business license and if having a trade name, an LLC, DBA, or corporation but you said you will use your own personal name.
